Would anyone be willing to review the attached op note and give me your recommendations on the CPTs to bill? My doctor wants to know if there are any other CPT(s) that would apply. I was looking at CPT 28737 as an additional code. I need help ASAP.

No on the 28737, there is no tendon lenghtening or advancement described in the op note.

You need an additional code for the 4 & 5 TMT resection arthroplasty but I cannot think of one that would be appropriate. It's almost like 28160 for the IP joint. I don't think there is one like this for the TMT. There are some arthrotomy codes but that's not what was described either. It's not really 28122 either. If you look at the 28050-28160 section it is like some of those. Whatever is chosen it would need a 59 (possibly unless unlisted) because it was done on a separate TMT from the fusion. You might have to go unlisted and cross it to 28160 or 28020.
