29823 and Open Biceps Tenotomy

HI everyone,

Looking for guidance on this operative report. Physician completed shoulder arthroscopy, debridement, capsular release and THEN and open biceps tenotomy. I am having a difficult time coming up with an OPEN procedure code for the tenotomy, any thoughts or suggestions would be great!!!! Please see op report below.

Am I able to code the 29823 with it? I was on the fence whether or not to add that code. Or would a mod 22 be added for the arthroscopic work done prior to the open?

Per his op report provider did not cut or released the tendon, he only debrided it. So this would not be appropriate code. Since patient was previously treated with biceps tenodesis and provider only removed scar tissue you would need code for that.

I would suggest the CPT 23430
