31254 help

I received a refund request for payment of CPT 31254 based on CMS guidelines CPT code 31254 was billed with place of service 11 which is not allowed in non facility location. Where do I find this guideline? My physician says that it may rarely be perfomed in a non facitlity location but that doesn't mean it can't and he did. He wants me to appeal this but I can't seem to find where CMS specifically indicated what can and can not be billied in a non-facilty location. Help please

I would contact your local Medicare carrier for guidance. I do know that Entellus is marketing cutting edge technology to perform these in house rather then in the ASC or Hospital.
