We are receiving numerous denial for the CPT code billed 36011 as inclusive to 93505 biopsy procedure. I would like to confirm that these two code are clearly bundled codes as the biopsy is the reason for the catheter placement.

Please advise...
 
36011 would need a 59 mod. if it's separate from the biopsy
