36561 / 36569 PICC Lines

Can anyone help with a diagnosis code for a PICC Line done thru general surgery with a 36561 or 36569? These are done for Chemo patients needing the port access. We have been coding them with the cancer code, and sometimes a 747.69 for anomaly of peripheral vascular system, but Medicare is now saying this does not support medical necessity. I have checked the LCD thru Novitas Solutions, and there is a very limited list of acceptable codes. Would appreciate any assistance!
 
We use V58.81 for CICC/PICC lines as primary. We haven't had any issues so far. Hope this helps.
I need to mention that the client is in Florida, so our MCR MAC is First Coast Service Options.
