Can 36591 be billed with 99213 (or any E/M) on the same day?

Can 36591 be billed with 96413 on the same day?

If so, do we attach any modifiers?
 
Port Flush

Port flushes can only be billed if it is the absolute only service performed during that encounter...they bundle into every other code and then some :)
