Wiki 38900 and 38792 done by surgeon

If the surgeon does both axillary lymphatic mapping using blue dye injection and radioactive sulfur colloid injection with sentinel lymph node biopsy.
Which CPT do you code 38900 or 38792?

Lymphoseek (tilmanocept) 125.0 uCi Tc-99m tilmanocept was injected intradermally at 4 sites in the left periareolar orientation x4, using standard radiation safety and sterile technique followed by injection of 5 mL of sulfan blue dye in a retroareolar orientation. Five minutes of breast massage was completed during prep and drape of both breasts and axillae.

We can code both 38900 & 38792. Please ensure that the Primary CPT is coded appropriately as 38900 is an add on code, which will not be acceptable without the stand alone CPT.



As per the NCCI edits, we should append modifier 59 with CPT 38792 as it has conflict with the add on 38900.
