if the report says fulguration to ablate the lesion by snare do you use code 45383 or 45385?

You need to ask the doctor because to ablate/fulgurate means to completely destroy. When you do this there is no tissue left to biopsy. Snare on the other hand is a method of removing a polyp and hopefully retrieving it to send for pathology. The sample of the op note you have provided is conflicting. What is ablated/fulgurated or was it removed by snare? I sincerely suggest you query the doctor for a clearer explanation.
