I am confused about when you would code 45390. Can this be used if it is a screening colonoscopy and the doctor removes a polyp by submucosal injection and hot snare???
 
The 2015 code set adds 45390 for flexible colonoscopy that involves endoscopic excision of mucosal tissue, meaning the mucous membrane lining the structure. The addition of 45390 is a part of a larger 2015 revision to the codes for mucosal resection. Keep in mind that this new code is for mucosal resection through a flexible colonoscope. Do not confuse this code with other new codes 44403, Colonoscopy through stoma; with endoscopic mucosal resection, and 45349, Sigmoidoscopy, flexible; with endoscopic mucosal resection.

In this procedure, the provider performs flexible colonoscopy along with excision of mucous membrane tissue using an endoscope.

Clinical Responsibility
When the patient is appropriately prepped and anesthetized, the provider inserts a flexible colonoscope to examine the colon for any lesion or lesions. After the identification of the lesion, she typically administers a submucosal injection to raise the lesion, making it easier to resect, or excise, it via snare or other means. Sometimes, the provider may repeat the injection in cases of deep lesions. After the excision of the lesion, she ensures hemostasis and removes the colonoscope.
