Polyp was raised with with injection of Everlift and demarcated. Polyp was partially removed in a peicemeal fashion by endoscopic mucosal resection . Residual polyp was removed with cold forceps and then ablated with APC.

Since addition steps had to be take to remove the entire polyp, can you code it as 45390?
