Can a 49000 bel billed with 58180? I see where it is included with Abd hysterectomy 58150, but I can't find it with Supracervicle hysterectomy 58180. Thanks
 
CPT 49000 is column 2 code for 58180 as per CCI edits, these codes cannot be together in any circumstances. So bill only 58180.

A good rule of thumb to remember is that anything that is a "separate procedure" can only be billed when it is performed alone (i.e. separately).
