Hi, can anyone help guide me in the right direction with this one. Patient came in and had lesions on hand, arm and thigh. Code 17110 was used for these lesions with 59. Office visit coded 99213-25. Then 54056 for 4 lesions was used for the penis lesions and was denied. Diagnosis code 078.11. Did the 17110 cover the 54056 or can I use the 54056 with a modifier? Thank you!
 
when i run those 3 codes (99213,17110,54056) thru our coding program, the only thing that needs a mod is the office visit (25) so maybe the problem is the 59 on the 17110?
