54450 Foreskin manipulation including lysis of preputial adhesions and stretching.

The lay description states: The physician treats adhesions between the uncircumcised foreskin and the head of the penis that prevent the retraction of the foreskin. Adhesions are broken by stretching the foreskin back over the head of the penis onto the shaft or by inserting a clamp between the foreskin and the head of the penis and spreading the jaws of the clamp.

My question is, if this is done in the office, and the patient is circumcised, can this code still be used since is states in the lay uncircumcised?

Also is this enough documentation to support:

Problem # 2: REDUNDANT PREPUCE AND PHIMOSIS (ICD-605)
cause, course, and treatment discussed
released and cleaned
use vaseline for few days to keep from re-adhering

54450 is used on an UNcircumsised boy, 54662 is adhelysis after circumsied and yes we do both in the office but a paragragh states exactly how they did the procedure.
