PT CAME IN FOR A SCHEDULED PHLEBOTOMY IN OUR INFUSION ROOM. THE PHLEBOTOMY WAS INCOMPLETE AS THE NURSE WAS ABLE TO ONLY REMOVE 55 ML OF BLOOD.
WOULD I STILL CHARGE FOR THE PHLEBOTOMY OR WOULD I CHARGE SOMETHING ELSE?

99195 is not a volume-specific code. I have never used a modifier when we removed less volume than planned. If you consider it to be an incomplete procedure, review modifier 52 and see if you want to report it.
