CPT does not mention this as applicable and you should not be following the Optum lay description in this case. The official CPT vignette for the work involved is as follows:
for 56620 intra service info: Visually identify the vulvar lesion and note its proximity to midline structures such as the clitoris, urethra, and/or anus. Make a skin incision around the lesion including a 1-cm margin if a preinvasive lesion is the indication. The excision includes epidermis, dermis, and superficial subcutaneous tissue. Obtain hemostasis with cautery and/or suturing. Then close the defect primarily in layers, with the first layer including subcutaneous fat and dermis and the second layer including skin. Apply dressings.
IF the closure required skin grafts you could also code (
