Hey there,

one of our docs with in the middle of a 58561 and got some "bites" of the fibroid but had to discontinue and ultimately just a 58558 was done would you code it as just 58558 or 58561 with a 52 or 53? here's the relevant excerpt from the surgery

The operative site of the MyoSure was placed against the smaller fibroid in hope to collect tissue for diagnosis. Some small bites come out but overall it was hard and not able to remove. Heavy bleeding was encounter so decision was made to forgo the rest of the procedure. A sharp curettage of the uterus was performed with tissue obtained for pathologic examination. The tenaculum was removed from the cervix with good hemostasis of tenaculum sites noted. The speculum was removed.

I would consider this 58561-53.
If the provider had tried once, and got nothing, then 58558, but that's not the case here. I am not aware of any "official" guidance that would make this definitively one way or the other. It's about how the existing guidance is interpreted. I wouldn't say 58558 is wrong, just that 58561-53 is more correct.
