I know this question has been asked many times. Please read this note, is it coded correctly?


The patient was brought to the operating room and placed in the prone position. A small pillow was
placed underneath the patient's abdomen in order to flatten the lumbar lordotic curve. The skin above the
region to be injected was cleaned with a surgical prep solution three times in the usual sterile fashion. A
sterile field was then created by draping around the prepped site. Using an AP view the appropriate
interlaminar space was identified. The fluoroscope was tilted caudally and rotated appropriately to best
visualize the targeted interlaminar space. The skin and subcutaneous tissue over the target area was then
anesthetized using 0.5% Lidocaine with a 25-gauge 1.5 inch needle. An 18-gauge Tuohy needle was
introduced under direct fluoroscopic guidance and directed towards the interlaminar space. A lateral
view was taken to guide the needle tip to the area superficial to the ligamentum flavum. The loss of
resistance technique using a pulsator syringe and preservative-free normal saline was utilized to get the
needle tip into the posterior epidural space. No paresthesias were noted from the awakened patient.
After negative aspiration for air, blood or CSF, a solution consisting of 80 mg of triamcinolone and 3
mL's of preservative-free normal saline and 1 mL 0.25% preservative-free bupivacaine was injected. The
needle tract was flushed with lidocaine and the needle was then removed.
A good loss of resistance as well as epidurogram was found at approximately 9 centimeters.
