63042 and modifier?

What anatomical modifier should be billed with CPT code 63042 for a reexploration of lumbar laminectomy L2-3, excision of herniated disc and extruded disc fragment? The ins co will not pay without this they said. Does anyone know?

This will be billed with RT, LT, or 50. The dictation should read what side the disc herniation was obtained.

Hi Mandy
If both sides of lumbar area receive the laminotomy lumbar nerve decompression use CPT 63040 with modifier 50 .Use the CPT 63042 if for one side of the lumbar area right or left done with LT or RT modifier. This is found in the CPT manual.
