Hello,

My provider and I are having a bit of a disagreement as to how to code this procedure:
"The holes for lateral masses screws made and tapped from C3-C7. The facet joints were decorticated. The lamina and spinous processes were removed from C3-C7 using the trough method bilaterally. Foraminotomies were then performed using Kerrison's rongeurs."

He is adamant that the procedure coding should be 22600, 22614*3, 63045, 63048*3. I think based on his documentation, CCI edits, and CPT descriptions he should only code 63045 and 63048*3.

Any help is appreciated and we are totally deadlocked as he has paid examples and I have denied examples of similar coding scenarios.

Was a graft placed? If an allograft or autograft was not placed to fuse the bones, code 22600/22614 would not be correct.
Code 22600/22614 needs decortication of the bone and placement of a graft alongside (lateral) the vertebrae.
