If our doctor does an L4,L5,S1 Laminectomy w/foraminotomy and facetectomy and a right L4-L5 discectomy and a left L5-S1 discectomy, would this be billed as 63047 & 63048 or 63030 & 63035?

The primary diagnosis given is the Lumbar stenosis and radiculopathy, but it is documented within the op report that the patient had disk protrusion and a herniated disk.

Any thoughts??
