Our doctor bills 63685, 63650, 63650-59. Both 63650 are denied because ASC are paid for those 2 codes. Denial reason: If you are the supervising physician, please resubmit claim indicating the appropriate Anesthesia (Q) modifier.

What Q modifier Aetna is talking about?
