How do you all get paid for bilateral percutaneously placed spinal cord stimulator electrodes? I have tried getting it paid using the below combinations but none of them led to a reimbursement. It keeps getting denied saying - payer deems the information submitted does not support this many/frequency of services.

63650, 63650-51
63650, 63650-59
63650, 63650-XU
63650, 63650-76

So this isn't a procedure that is technically billed as bilateral. You are only going to bill 63650 X 1 for each lead/electrode placed. Most often, two leads are placed. So you will only bill 63650, 63650-51. 63650 has a bilateral indicator of "0" which means the following:

Bilateral indicator 0​

-Services may be unilateral, or another procedure code exists for services performed bilaterally.
-Procedure code should not be reported with a 50 or RT/LT modifier.
-If reported bilaterally only one unit will be considered for payment.
