Hi, my claim was rejected by the payor. I billed 00800 P3 with 64486 59
the payor is telling me it is part of the standard preparation. what can I do to prove it was separate.
 
Which payor?

Do you have a separate Procedure Note for the injection? That is what we use to appeal these rejections. We normally get them from Humana and UHC. but we appeal with our separate procedure note which noes the start and end time being different than the anesthesia surgery time billed.
