Wiki 64898 -only bill once?

Would someone be willing to look at the attached op note? CPT 64898 is Nerve Graft, multiple strands, arm or leg up to 4 cm. Does that mean that it can only be billed once. My doctor is asking if we can bill times 5 since in the last paragraph of the op note it states: The fascicles were then teased apart into 5 branches, each branch was then repaired to its distal fascicular branching, requiring interval resection of the neuroma.
Any help is much appreciated.

I didn't read it so not sure of coding, but the MUE on that CPT 64898 is 2 (once per arm or leg). I don't think 5 would fly. It's for multiple proximal and distal ends. I believe that's why the description and code is plural and has the word multiple.
