In the event 69210 is performed and it is NOT the sole reason for the visit therefore only the appropriate e/m code applies, is 69210 still supposed to be included in the list of procedures completed on the 1500 form even though no reimbursement will occur?

Why would 69210 not be reimbursed? If it was truly done it should be documented and billed. The E/M would need a -25 modifier so to signify it was seperate from the visit.
