My supervisor is looking for documentation to explain what is significant time and effort and how to document it. I have already given her the info on what constitutes impacted cerumen and the poicy info regarding what instrumentation needs to be used. Does anyone know where I can find what they mean by significant time and effort?

Significant Time and Effort?

Removal of impacted cerunem is not a time or effort based code. The description basically identifies instrumentation necessary and rules out irrigation as a method. If the provider actually removed the impacted wax using instrumentatio
n and didn't simply wash out the ear, that should suffice as documetanation.

Also "impacted" must be documented as well as method of removal.
