Hello, I have an orthopedic DR who did an US-guided botox injection for piriformis syndrome, bilateral hips. Per the LCD, G57.01 and G57.02 are not covered. Any suggestions on what code I could use?

Was it US guided or Fluoro? Did they use Botox or another steroid? M53.3 is an acceptable code. It's hard to know without more info.
 
Because Botox is used you would bill it with 64646 Chemodenervation of trunk muscle. Check the CPT Book (sorry don't have it with me at this time) to see if ultrasound is allowed. I know that Fluoro is included with the 64646. As far as dx I would check the LCD's of the insurance.
