Our doctor is doing both post op nerve blocks under ultrasound guidance & PRP injections. My questions is what are the requirement for billing for 76942, I know we must have a hard copy of the image, but if someone can maybe give me an example as far as documentation goes? (interpretation)?

As per my understanding, hard copy imaging is must for 76937 not for 76942. We code 76942 for the documentation which states "Ultrasound guidance was provided for the Dr.____. The image shows ..(diagnostic findings).

Don't append 52-modifier for 76942.
