Does anyone having information on billing 77002 with 20610. I have tried to research this unsuccessfully. Medicare is now bundling

20610 is listed as a parent to the add-on 77002 in the CPT book.

Where was it performed? (Was a 26 mod needed if in a facility?)
What was the denial reason code? Was the 20610 paid?
Was everything on the claim coded correctly, the 20610 was not mistakenly 20611, etc., what were the units?
What was the anatomical location and what medication was injected?
