I am billing 77470 for patients getting concurrent chemotherapy with external beam radiation. Is the phrase below enough documentation to show medical necessity and support the billing of 77470?
(Patient) will be receiving concurrent platinum based chemotherapy which will increase a variety of treatment related side effects, most notably pharyngitis.
