We are billing 88291 with a 90 modifier because the CYG test are send outs. Medicare is rejecting this code for missing/invalid modifier. Any thoughts?

Are you sure the lab where the test are being sent to isn't billing? I thought you would have to provide the service before you can bill for it.

Do you have an ABN? Medicare sometimes requires the use of GA ~ meaning that you have a ABN on file for this test. If not you will need to pick a different modifier for this.
