GI doc is wondering....when a malignant polyp is removed from the colon endoscopically either by biopsy or snare, and the pathologist has to report margins based off of the polyp, which CPT code should we use for pathology? 88305 or 88307? Remember it is just the polyp not a colon resection. Also what about an Endoscopic Mucosal Resection? 88305 or 88307 for pathology?

What you describe would be 88305, as it is less than a resection of the colon. It would be 88305, regardless of the final diagnosis and the number of slides or blocks.

Choosing the correct CPT code has less to do with the procedure it came from and more from the type of tissue received. 88307 assumes the tissue represents a partial colectomy (segmental resection, other than for tumor), which may be small or large in size. You should check with your pathologists about particularly small resections (less than 5 or 6 cm) as they may determine that the work for these very small resections is more accurately represented by 88305.
