In the below example, which cpt code, 88360 vs 88342 would be coded for the ER and HER2?

Breast Biomarkers

Estrogen Receptor (ER):
1. Negative (0%)

Progesterone Receptor (PgR):
1. Positive (greater than 90%)

HER2 by immunohistochemistry): Negative (score 1+)

All three should be billed with 88360.

That is how we bill those out where I work.

Referring to our CPT book - it states Morphometric analysis, tumor immunohistochemistry (eg, Her-2/NEU, estrogen receptor / progesterone receptor), quantitative or semi quantitative, per specimen, each single antibody stain procedure; manual

However if you are using computer assisted technology it would be 88361 x3.

Per our laboratory catalog it states: Results are reported as negative (0, 1+), equivocal (2+), and strongly positive (3+) according to the interpretation guidelines for the FDA-approved Ventana Pathway HER2 (4B5) antibody.

agree with danachock, even the HER2 should be 88360 since they are giving a score...if it was just positivie/negative than the 88342 would be appropriate, but the 1+ bumps up to 88360
