We are billing 90734 Menveo (Glaxo Smithkline) with an NDC of 1 unit. BCBSM of Michigan is only reimbursing half of the allowed amount stating the NDC allowable quantity is 0.5ml. Is anyone else having this problem and/or are you billing this differently?
