Can I bill a 90792 when video was not available, and visit was audio only?

Per CMS Medicare allows 90792 as audio-only. But you will need to check with other payers, for example, Indiana state Medicaid does not allow audio only for this service. Modifiers are required for telehealth billing in most instances. Here are the descriptions from the 2025 HCPCs book : 93 Synchronous telemedicine service rendered via telephone or other real-time interactive audio-only telecommunications system; 95 Synchronous telemedicine service rendered via a real-time interactive audio and visual telecommunications system.

Appendix T of your CPT manual contains a list of codes that can be audio-only. However, you need to check the payer for its policies on what it does/not allow as an audio-only service.
