I understand this code is no longer used however for past years, what is the minimal documentation requirements needed? Should the treatment plan be referenced? Should goals and objectives be minimally talked about, should a reference to current medications and why they are taking them be explained? Total duration of the service?

Please help. Any examples of progress notes for billing of 90862 would be very, very helpful. Thanks!!
