Why would Horizon deny my claim for 92012 due to diagnosis?

Patient was seen one month prior with and we billed with the same diagnosis. Could that be the issue?

Have you checked to determine whether they have a limit on the number of times 920xx codes can be billed per calendar year? We have encountered that with a vision plan, where they will only pay one per calendar year, regardless of the dx.
