I just received some information from our rep last week. The information says for physician's to use CPT 33975 for insertion and 33977 for removal. According to the rep we are allowed to bill for both the insertion and removal.
Since we just did these a few weeks ago I can't tell you how they pay but this is how I am billing them.
You might check out Abiomed's website and see if they have printouts you can use.
