Thank you for all this information. This is a new code for our practice and I've been tasked with gathering information on it. Now that I've figure out which jurisdiction I'm in, is there a list of approved codes somewhere online or is it noted next to the code in the CPT book?
You’re welcome. The CMS website is a great source of reference for what you’re looking for. The CPT book does not provide payer specific guidelines. That knowledge typically comes from work experience in a billing department/ back end revenue cycle. Here’s a link from CMS.gov that you can navigate to help find the info you’re looking for

This above link is more related to the Stark Law and that list is the list of DHS as related to Physician Self Referral. It's not necessarily a list of "covered" CPT.

I think you might be looking for NCDs/LCDs possibly? https://www.cms.gov/medicare-coverage-database/search.aspx

Here's an example for an Anthem MCR plan that states it's non-covered: https://blue.regence.com/medicare/med/m-med168.pdf
myocardial strain imaging component (add-on code 93356) is subject to the criteria noted below and denies as a nonreimbursable service (NRS) for Medicare Advantage.

Here's another Anthem plan that says it's investigational: https://al-policies.exploremyplan.c...t_publisher/gvKEs0SDu27L/content/mp-726/78515

Here's a webinar about it:
