Does anyone know if there is a limit on how many 95812 can be billed within a time frame? My provider uses this code for a Quantitative EEG and biofeedback for behavioral health. We submit approximately two 95812 each week per patient and these patients treat 3 x per week. We are receiving a UPIC denial N640 exceeding the number/frequency approved/allowed within a time period. I cannot find anything online about limits, such as 180 days or once yearly. I've called my CA MAC, and the reps tell me a limited amount can be billed. Does anyone know what that limit is?
