Wiki 95990 for a programmable pump?

In the CPT Assistant dated August 2012 it said programmable pumps require programming when they are refilled. So based on that it seems that any refill of a programmable pump should be billed with one of the codes that includes programming such as 62369 or 62370. However I see providers billing 95990 or 95991 (refill and analysis, without programming), when the pump is definitely programmable, such as a Medtronic Synchromed II.

Does anyone know if programmable pumps do actually require programming at every refill? Is it possible that the 2012 CPT Assistant was incorrect or has something changed since then? Just wondering why I keep seeing 95990 or 95991.

95990 and 95991 are used for non-programmable pumps (constant flow or reservoir).
