If a teacher completes the form and faxes it in to the provider, is the provider able to use this code on the visit? Or do they actually have to conduct the screening?
 
If the provider reviews the teacher form in office at the visit of the patient, they can use the 96127 code. If he reviews more than one you would use a modifier 59
