Can you direct me where to look for CMS approved ICD 10 dx codes to bill with 96164 and 96165 codes?
 
Did you try going to the CMS coverage database and entering 96164? It doesn't appear they have an NCD and only a few LCDs but you should be able to use the published LCDs as a starting point if one of them isn't your MAC.
