Can someone explain to me (again) what the requirements are to bill monthly B12 injections.
96372 has a blurb in CPT regarding Direct supervision requirements. I have found some notices regarding the definition of Direct Supervision, but I also see a few revisions. What are the Up to Date direct supervision requirements?

If it is Routine for the Nurse to give such injections can we bill 99211.
