Can anyone furnish a descriptive statement they've used when submitting a claim for this CPT code? Even though we're aware that a good portion of our payers will not pay this code, I'd like to use a well crafted sentence that would provide the proper documentation that would satisfy payer requirements? Yes, I'm prepared to call each payer if necessary, but would like to extract from a common consensus from my peers on here, please!
