Help! I was told a few years ago not to charge a 99135 with my doctors when they have Anthem, Medicare, or Medicaid by my (no longer) senior coder! I have found that Medicare and Medicaid does not reimburse this but I cannot find anything on Anthem. Please help! I'm in a panic mode!
