If the problem is low, data is minimal, and mdm is moderate, do I go in the middle and go with 99203?

The level of MDM is "based on 2 out of 3 elements of MDM" - consult the AMA MDM tool: https://www.ama-assn.org/system/files/2019-06/cpt-revised-mdm-grid.pdf
You're correct, Low/Minimal/Moderate = Low MDM = 99203
