Can a doctor code 99211 or is this code for other staff like nurses, MAs etc?
 
absolutely the doctors can bill a 99211 - typically they don't though! ... more often they are associated with a "nurse visit"... minor visit.. BUT, the doctor certainly can bill it. I assume the visit was very minimal and that's what the doctor decided was supported by the documentation.
