Wiki 99211 vs 99212 for Procedures

I guess the short answer is that it's appropriate to change the code when the documentation supports it. But the questions I'd have to start with are why is the practice management system defaulting a code, and why is it choosing 99211 for a procedure? Electronic systems are notorious for choosing codes incorrectly and it's best to avoid automatically using what the systems suggest. 99211 is for evaluation and management services, not for procedures, with perhaps a few exceptions. Can you give any examples of what is being coded this way?
