Wiki 99218 and 76830

I have a provider who is billing and observation code and an ultrasound 76830 w modifier 26 and the insurance is denying the claim.. How can I get this claims to be covered.

You might consider adding modifier 25 to the E&M code if the visit entailed more than just reviewing the US. I hope this helps.
